Pulaski Music Booster College Scholarship - Student Application
Note: You must be a senior currently registered in band or choir at Pulaski High School to fill out this application.

Criteria for consideration for a scholarship from the Pulaski Music Boosters:
The applicant must:
1.  Be a senior at Pulaski High School who is currently registered in band or choir.
2.  Major in Music at a College or University in the fall
3.  Provide recommendation letters from three people: one of which must be a Pulaski High School music teacher, and one from someone outside of music.
4. Mail recommendation letters, no later than April 10, 2010, to: The Pulaski Music Boosters, C/O Roy & Tyra Smith, 128 Fourth Avenue, Pulaski, WI, 54162
5.  Complete and submit this application form by April 10, 2010
6. Late and/or incomplete applications will NOT be accepted.
